I will exhibit responsibility, integrity, and a Christ-like attitude.
I commit to serve the Lord and His children to the best of my ability.
I will prepare ahead of time for my VBS responsibilities.
I will be present at my required time.
I will actively facilitate and participate in all aspects of the VBS program.
I understand that VBS is a Children’s Program. I will be there for the children. Crafts and toys are meant for participants' use. Their safety and enjoyment will be my primary focus.
I understand that appropriate and modest dress is required. Jeans and shorts are permitted, but they must be of a modest length.
I will help clean up my area of responsibility on Friday afternoon.
I give permission for my image to appear in photographs and videos for future promotions.
If at any time I provide an email or text address to Cornerstone Chapel at which I may be contacted, I consent to receiving communications/information at that email or text address. Cornerstone Chapel does not charge for this service, but standard text messaging rates may apply as provided in your wireless plan (contact your carrier for pricing plans and details).

I have read and agree to the above commitments and desire to serve in Vacation Bible School.

For parents/guardians of minor age participants (under 18 at time of registration):
I, as parent or guardian for minor participant, do release, acquit, discharge, and covenant to hold harmless and indemnify Cornerstone Chapel, its representatives, their volunteers, activity leaders and assigns from any and all injuries, disabilities, actions, damages to persons or properties, and liabilities arising out of any incident, accident or sickness, or treatment thereof, incurred by my minor child’s participation in this program and/or activities, to the fullest extent permitted by law.
